Although the story was inspired by a conversation with a friend back in the day, the title was inspired by an episode of Hana yori Dango, "Kare to Atashi to Aitsu." Of course that does not translate easily into English, the closest probably being something more like, "Him and Me (or He and I) and That Guy." If that sounds better, that would probably work just as well. Another possibility I thought of once was "He and I and Him," but I wasn't sure whether the use of the inconsistent grammar would really convey the right meaning.
